MoHP: What to do if you have lost your vaccination card?

The government has made vaccination cards mandatory for entering public places and seeking services from January 21.

And in addition to that, the Ministry of Health and Population (MoHP) has made it mandatory to have a vaccination card to get a booster dose as well. 

So what to do if someone has lost their card? There are a few ways one can obtain their card as per the ministry. 

  • The ministry has said that someone who has already been fully vaccinated can get their copy of the vaccination card from the vaccination center where they got their second dose from. Same goes for those who have obtained only one dose, they can get the copy from the health center as well.
  • Similarly, the ministry says to get vaccination cards for those who have been vaccinated with Johnson & Johnson, they can go to the health department of the municipality where the health center or institution is located.
